Located in the rural heart of New Hampshire, Mid-State Health is committed to providing evidence based, top-notch primary care with a patient-centered approach. Our aim is to help the residents of our communities lead healthier lives while making the most of their healthcare resources. At Mid-State Health, our mission is simple: to offer quality primary healthcare to all members of our community, regardless of their ability to pay.
We believe in treating the whole patient, not just their symptoms. Through our medical home model, we’ve seen improvements in health outcomes, cost reductions, and increased satisfaction among patients, families, physicians, and staff alike.
