We believe cost should never be a barrier to receiving care. That’s why we offer several financial support options, including our Sliding Fee Discount Program, available to eligible patients regardless of insurance status.

Step 4: Prepare for Your First Visit

Your first visit is an important step in building a lasting relationship with your care team. To help us provide the best care possible, please bring:

  • A valid photo ID

  • Your current insurance card

  • A list of current medications and supplements

  • Any relevant medical records
