Making medical care decisions is not always simple. It is very important to ensure that your health care decisions and wishes are clearly noted and communicated when you are not able to speak for yourself. It is your right to participate and plan for your care. It is also very helpful for family and your health care team. Advance Care Planning provides guidance about your wishes about treatments you would or would not want if you are unable to speak for yourself.

This is particularly important in New Hampshire where there is no law to provide your relatives the legal authority to speak for you about medical decisions if you cannot speak for yourself and you do not have advance directives.
