Preparing For Your First Visit

Welcome to Mid-State Health Center, where we are committed to providing the best care possible for our patients. If you are a new patient, we are excited to have you join our practice. Please see below for some important information regarding becoming a patient with us!

 

Appointments:

To schedule an appointment, simply call our office, and our friendly staff will assess your needs and schedule you appropriately. We understand that emergencies happen, and same-day appointments are often available. If you need to change or cancel an appointment, please notify our office immediately.

 

What to Bring with You and When to Arrive:

To ensure that your appointment is productive, we recommend bringing all documents that you feel are important to your visit, including:

  • Recent healthcare services received outside of Mid-State
  • Drivers license or other valid photo ID
  • Your current medications / supplements
  • Your insurance card

Please plan to arrive 15 minutes before your scheduled appointment time to allow time for the check-in process.

 

Prescribing Medications at Your First Visit:

At your first visit, your provider will review your existing health conditions, medications, and previous treatments. Your health is very important to us, and your provider will discuss these factors with you to create a plan that best meets your health goals.

If you will be out of medication before your first office visit at Mid-State, we ask that you contact the provider who wrote the original prescription. Often, you may be able to get a bridge prescription to cover the short period of time as your transfer your care to our practice. This is at the discretion of the provider who wrote the prescription.

If you are currently taking a controlled substance, Mid-State is required to look up your prescription history in a database called the Prescription Drug Monitoring Program (PDMP). The results of this search may affect your prescription for controlled substances and a new contract may be required.

Due to Federal Laws, Mid-State’s providers are NOT able to write prescriptions for medications with tetrahydrocannabinol (THC) which is in medical marijuana.

 

Services to Ensure Your Visit is a Great Experience:

At Mid-State Healthcare, we offer various services to make your visit a great experience.

  • 24-Hour Access to Advice: Our on-call clinicians provide Mid-State patients advice by phone for urgent health concerns 24-hours a day, 7-day a week. Your health and safety are our top priority. There could be times when you may be advised to go to the nearest Emergency Department instead of coming to the office.
  • Patient Portal: You may contact a member of your care team by sending them a message through our Patient Portal. You can also use this service to access your medical files, appointment history, and more. Please ask the staff at check-in to sign you up for this service.
  • Interpretation and Language Services: Mid-State will provide an interpreter for our patients as needed at no cost. Please let our office know ahead of time so we are able to plan accordingly.
  • Prescription Coordination Services: Mid-State‘s Prescription Team can assist you with questions about your medications, refill assistance, affordable medications, and transferring prescriptions.
  • Transportation to your Visit: If you need assistance with transportation to your visit, let us know! Often, we can help you coordinate a ride to and from your visit through our van or another transportation service. 
  • Completing Forms: If you would like assistance in completing your forms, we are happy to help. Simply call us to schedule a time to meet with a member of our team.
  • Managing the Cost: Mid-State offers assistance in enrolling in a variety of coverage options, prompt payment discounts, and offers a sliding fee scale to those who qualify. To begin the eligibility process, please call our Patient Account Representatives at 603-536-4000 ext. 1404 and they will gladly assist you.

 

When You Arrive:

We understand that visiting a healthcare provider can be stressful, so we strive to make your experience as comfortable as possible. Our patient services team will check you in, confirm your information, and ensure that all necessary paperwork is complete.

A medical assistant will then call you into the clinical area, and lead you to an exam room to take your patient history, vital signs, and review any current conditions and medications. Your provider will then come to meet with you for your visit. After your visit, please check out with the patient services team, who will give you important paperwork about your visit and arrange any referrals, labs, services, or other visits you may need.
