Meet the Patient & Family Advisory Council (PFAC)!
What is PFAC?
Mid-State Health Center is pleased to introduce its initiative to promote and improve our patients’ involvement in primary care, the “Patient Family and Advisory Council” (PFAC).
PFAC is a group of patients, caretakers, family members and staff who are committed to improving the quality of care we deliver. Council members share their perspective and provide meaningful feedback on a wide variety of topics affecting the patient and family experience.
This group meets, at minimum, every other month on the third Tuesday from 5:30 to 7:00 PM in the Plymouth office. Formerly called the Patient Expert Advisory Team, or PEAT
