SLIDING SCALE – COMMUNITY CARE
Services at Mid-State are available on a sliding scale to all patients who qualify. Please feel free to contact our Patient Accounts Representatives for more information.
Our Patient Accounts Representatives provide assistance for patients who are uninsured or underinsured and wish to apply for a sliding fee scale at Mid-State. Patient Accounts Representatives are available to provide assistance with Community Care Application, Health Care Marketplace Application, prescription assistance, payment plans, and other programs to assist you. The goal is to help make your health care costs here at Mid-State affordable.

Mid-State Health Center strives to make accessing health care as smooth a process as possible. If your insurance company is not listed, claims will be submitted. However, we cannot guarantee coverage or payment.
Patients are responsible for all outstanding balances not covered by insurance.

SELF-PAY PATIENTS & UNINSURED
We are happy to work with patients who are self-pay. We offer assistance in accessing enrollment in the Health Insurance Marketplace. Self-pay patients that pay at the time of service will receive a 30% discount (discount is not available for dental services or sliding scale fees).
NH HealthCost was developed by the New Hampshire Department of Insurance to improve the price transparency of health care services in New Hampshire.
Visit their website if you would like to explore what you might expect to pay for certain services based on where you go to get the service and what type of service it is. This includes things like MRIs, Colonoscopy, Mammogram, and many other health services, tests, and screenings.

Outside of Open Enrollment, you may qualify for a Special Enrollment Period:
In the Marketplace, you qualify for a special enrollment period 60 days following certain life events that involve a change in family status (for example, marriage or birth of a child) or loss of other health coverage. Job-based plans must provide a special enrollment period of 30 days.
