Pre-Registration

At Mid-State Health Center, we understand that your time is important!

We now use an online system, Phreesia, that allows you to pre-register before your visit. Using your smartphone, tablet, or computer, you can access the secure portal to confirm your appointment and update your information at your convenience. There is no special software or app required!

This will help us securely collect the most up-to-date information, reduce our paper consumption, and reduce time spent in the waiting room.

Call to make sure we have the correct cell phone number or email address on file for your family! 

How It Works

 Before Your Visit

 You will receive an email or text three days before your visit from Mid-State via Phreesia with a reminder about your appointment and a link to pre-register. You cannot pre-register online without this unique link. 

 When you click the link, you’ll be asked to confirm your identity with a phone number and date of birth.

 Mobile pre-registration is available in 20 different languages, and in office PhreesiaPads are offered in English and Spanish. 

 Once confirmed, you will be led through a series of questions that may include:

  • Your contact information
  • Other personal information, such as employment and marital status
  • Contact information for the person responsible for your bill
  • Emergency contact details
  • Insurance information
  • Consent for treatment and other forms
  • Questions about your health to help us evaluate your needs
  • Copays or balances due ahead of time

 You will be given the option to confirm or update the information we have on file for you. You may also be asked to upload images of your insurance card(s) and photo ID.

By completing your registration ahead of your appointment, you’ll save time and limit discussion of your personal information at the front desk when you arrive.

At Our Office

 

Please come 10-15 minutes early to your appointment and bring your ID and insurance card. If you’ve completed your pre-registration before your arrival, just check-in at the front desk to let us know you’ve arrived.

 If you are not able to complete the pre-registration before your visit, please arrive early so that you can complete this in office.

 

What is Phreesia?

Mid-State has partnered with Phreesia to give you a digital way to connect with us for your visits, including mobile registration. Mobile pre-registration allows you to complete important health information from your own device (any smartphone, computer, or tablet) ahead of time.

Mid-State is partnering with Phreesia to save you time when arriving to our office and to ensure that your health records will always be up-to-date. The information you enter is private and secure and will allow our team to better care for you.

Is the Phreesia system secure?

Do I need to use this system for every appointment?

Yes. But once you have completed the initial registration, which takes about 10 minutes to complete, the next time will be much quicker because your previous answers will be saved. 

Do I need to download an app to do mobile pre-registration?

No. There is no app – you simply click the link provided in the text message or email. This allows you to securely answer any registration and medical questions necessary before your appointment. 

What happens if I can't finish my pre-registration before my appointment?

That’s okay! If you do not register before your visit, please see our Patient Services Team and they will be happy to either send you a link to your phone or provide you with our in-office PhreesiaPads. 

Don’t worry, Phreesia saves the information you entered previously.
