At Mid-State Health Center, we understand, acknowledge, and respect any individual’s right to privacy and the concerns one may have concerning privacy and security. We recognize the importance of protecting the privacy of information provided by our patients and general users of our website.

We will not disclose, sell, or rent your name or any personal information about you to any third party. Income, demographic, and non-identifiable clinical data may be shared with quality and accreditation organizations as required by law, as well as to receive State, Federal, Private, and Research grants.

Do not rely on electronic communications or this website for assistance with urgent medical needs or to communicate patient information. No patient information of any nature should be shared through this website. Patients should use the Secured Patient Portal for communication or call the office directly. This website is not designed to help communicate or respond to medical emergencies. Mid-State Health Center cannot guarantee any response to a medical emergency or patient information communication if done so through this website.


You do not need to give personal information to receive information from Mid-State Health Center’s website. All visitors can browse and access our entire site without providing any personal information. We do not collect personal information that would identify our visitors unless you provide it, such as through a job application, charitable donation, or sign up for our e-newsletter.


Our website may contain forms through which individual users may request information, register for events and classes, or supply feedback to us through a survey. In some cases, we require telephone numbers, email addresses, or return addresses so we may supply you with the requested information. When processing credit card payments, correct names and addresses are also required.

We do not provide any information supplied on our web forms to any outside organization for any reason (other than when required by law or State & Federal organizations or as necessary to process credit card information).


Surveys

To better communicate with our patients and to improve our patient care and services, we may survey visitors to our site. We use the aggregate information we collect to measure the number of visitors to our site’s different pages and sections and to help us make our site more helpful to visitors. The aggregate information we collect is not linked to personal information, and we do not report on individual user sessions.


Online Donations

Those who pay their bill online or donate to Mid-State Health Center through our website are directed to a secure portal to give their information. We will not collect your information directly.


Email Inquiries to Lamprey Health Care

Our website contains several email links that allow web visitors to quickly send an email inquiry or feedback to Mid-State Health Center. When you use the email links, your email address and the subject of your inquiry are provided to the department to respond to your specific request only.


Username and Password

If the Mid-State Health Center website provides access to any service requiring a Username and Password, you are solely responsible for keeping your Username and Password strictly confidential.


Non-Personal Information

Mid-State Health Center collects non-personal information such as website usage, site traffic through visits, site performance, and related statistics based on our use of Google Analytics to track individual visits to our website anonymously. Only visits are tracked, not the individuals who visited the site.


IP Addresses

The web server automatically collects the IP (Internet Protocol) address of the computers that access our website. An IP address is a number assigned to each user’s computer when you access the Internet. We use the aggregate information collected to measure the number of visitors to our site’s different pages and sections and to help us make our site more useful to visitors. The aggregate information we collect is not linked to personal information, and we do not report on individual user sessions.

Please Note: Although such information is not personally identifiable, we can determine from an IP Address a visitor’s Internet Service Provider and the geographic location of their point of connectivity.


Cookies

A cookie is a small amount of data transferred to your browser by a web server and can only be read by the website that gave it to you. It is not a program and cannot deliver viruses. The Mid-State Health Center website does not currently use cookies; however, to improve our site on an ongoing basis, we may or may not use cookies in the future.
