
Mid-state Health is excited to welcome Hailey Patridge, CHW, to the team as a Community Health Worker. With a well-rounded background in behavioral health and social services, Hailey brings both passion and experience to this important role.
Hailey is currently pursuing a degree in Psychology from Southern New Hampshire University and completed Criminal Justice Studies with a minor in Addiction Studies at Keene State College. Her education and professional experience have prepared her to work alongside individuals and families facing complex challenges, helping them navigate systems that can often feel overwhelming.
“Community Health Workers play a vital role in bridging the gap between healthcare systems and the communities we serve,” said Debbie Guillbert, Director of Compliance, Quality, and Patient Relations, at Mid-State Health. “We are excited to welcome Hailey to our team and are confident that her dedication and passion for community support will have a positive impact on our patients.”
In her role as a Community Health Worker, Hailey will work closely with patients, families, and community partners to help individuals navigate healthcare services, access essential resources such as insurance, utilities and housing assistance, food access and overcome barriers to care. Her work will focus on education, advocacy, and connecting community members to programs that remove barriers to care and support healthier lives.
“I am excited to join a team that is passionate about ensuring patients receive the support they need beyond clinical care,” said Hailey Patridge, CHW. “I look forward to connecting with the community and helping patients understand the resources available to them.”
Hailey is now available and ready to assist with a range of needs. She will be traveling between our Boulder Point, Bristol, and Lincoln locations, and can also travel to other Mid-State Health sites or community centers to meet patients where it’s most convenient for them.
