Mid-State Health Center Offers Chronic Care Management (CCM) Program

Mid-State Health Center is pleased to offer Chronic Care Management (CCM), in partnership with Precision Value Management, to assist Medicare patients in managing their chronic conditions.

CCM is a critical component of primary care that contributes to better outcomes for patients that have two or more chronic medical conditions with the support of their healthcare team. Chronic medical conditions such as diabetes, heart disease, and arthritis can be challenging to manage on your own. Our CCM program provides personalized care to help manage these conditions more effectively.

This program is led by a team of experienced healthcare professionals, includes doctors, nurses, and other healthcare providers who work together to provide comprehensive care.

With CCM, you can expect the following benefits:

  • Assistance with medication management and prescription refills
  • Help with coordinating specialist appointments and other medical services

To be eligible for CCM, patients must have two or more chronic medical conditions that are expected to last at least 12 months or until the end of your life, have Medicare Part B coverage, and be willing to participate in the program.
