Mid-State Health Center offers Curbside COVID-19 Testing in our Plymouth office by appointment only. Testing is available to everyone in the community, regardless if you have symptoms or not.
Scheduling an Appointment at Our Plymouth Office
If you wish to be tested for COVID-19 in our Plymouth office, please fill out the COVID-19 Testing Request Form.
If you are unable to complete this form, please call our office at 603-536-4000 to schedule a testing time.
The COVID-19 Testing Line is open Monday – Friday, 7:30 AM – 12:00 PM.
Please note, we have experienced an increase in call volume. If you call outside of these hours, or get our voicemail, please leave a detailed message with your phone number and email address and a member of our team will return your call.
We follow evidence-based guidelines for testing based on the Centers for Disease Control and Prevention (CDC) and the New Hampshire Department of Health.

Ready Today PCR Testing
Reverse Transcription Polymerase Chain Reaction (RT-PCR) testing is required for many travel destinations. Please make sure to check travel requirements prior to getting tested.
Test results are available same day.
Results are available digitally via the Mid-State Patient Portal. View the instructions to receive your results here.
RT-PCR testing is not billed through insurance.
Rapid COVID-19 Antigen Testing
This is an antigen test using a nasal swab (short swab, minimally invasive, in your nose) test.
Test results are available at the end of your appointment (approximately 15 minutes).
Pricing and Payment Information
Please Note: You may be charged for an office visit for your COVID-19 Test even if the visit is curbside.
Generally, most insurance companies cover the cost of COVID-19 testing when symptoms are present. We recommend you reach out to your insurance provider to confirm coverage prior to testing. We will submit claims to your insurance, however, we cannot guarantee coverage or payment.
If you do not have insurance or have a health insurance plan that does not cover the cost of testing, testing is still available to you. Please call us at 603-536-4000 to talk to our staff about your options.
INSTRUCTIONS FOR YOUR SCHEDULED APPOINTMENT
If you are scheduled for a COVID-19 test at Mid-State Health Center Plymouth:
- Do NOT use our main entrance
- COVID-19 Testing parking is located on the left side of the building. Upon entering our parking lot, look for the main signs and green cones and take an immediate left into the small parking lot.
- Park in an available spot marked with an orange traffic cone.
- Please stay in your car.
