The COVID Equity Program Coordinator has two primary functions:  as a Community Health Worker will assist community members in connecting with the appropriate social and health need resources to ensure equitable access to care, particularly focused on COVID-19 supports; and as the COVID-19  Equity Program Coordinator will oversee the implementation of the COIVD Equity Program at Mid-state Health Center acting as the lead and liaison for this new community-based health initiative. This is a benefited Fixed Term Position.

 ESSENTIAL FUNCTIONS:

Community-Based Health Advocacy (Community Health Worker – CHW):

  • As a Community Health Worker will connect community members to culturally and linguistically competent COVID-19 testing at community testing sites.
  • Will assist with contact tracing for community members, when required.
  • Provide cultural mediation among individuals, communities, and health and social service systems.
  • Offer culturally appropriate health education and information.
  • Provide care coordination, case management, and system navigation to community members.
  • Provide coaching and social support by advocating for individuals and communities.
  • Provide direct services to clients with COVID-19 and their family members affected by COVID-19, which include, but are not limited to facilitating access to testing and COVID and influenza vaccination, COVID prevention and vaccine education, and translation services.
  • Provide social determinants of health related services, by creating connections between vulnerable populations and healthcare.
  • Providing appropriate care coordination, case management and connections to patient and family identified community and social services and referrals.
  • Assisting with maintaining and/or applying for social services within their community.
  • Identifying and helping to mitigate barriers in health care access such as transportation, language, and childcare.
  • Assisting vulnerable populations with navigating the healthcare system.
  • Determining eligibility and enrolling vulnerable populations in health insurance plans.
  • Providing culturally appropriate health education on topics related to COVID, chronic disease prevention, physical activity, and nutrition.
  • Providing informal counseling, health screenings, and referrals.
  • Connecting clients with community-based agencies through closed loop and/or warm hand-off referrals for supports that include, but are not limited to:  food insecurity; mental health supports; health care referrals; substance use disorder supports; educational supports and services; financial literacy; budgeting and supports.
  • Increasing cultural competence among healthcare providers serving vulnerable populations by providing services that include, but are not limited to:
    • Educating healthcare providers and stakeholders about community health needs.
    • Managing care and care transitions for vulnerable populations.
    • Advocating for vulnerable populations or communities to receive services and resources to address health needs.
  • Completing data tracking system forms to document the care coordination and case management of the patient and family.
  • Will become a Certified Application Counselor with 90 days of employment to assist patients with enrollment in Healthcare Marketplace Insurance options and Medicaid.
  • The CHW will participate in at least one (1) professional development activity per year related to culturally and linguistically appropriate services and organizational cultural effectiveness.
  • The Coordinator/CHW is required to participate in CHW trainings and NH CHW Coalition meetings and conferences, as directed by the funding agencies.
  • Perform other duties as assigned.

COVID-19 Equity Program Coordinator:

  • Oversee the implementation of the COVID 19 Equity Program in alignment with funding agency guidance.
  • Responsible for creating and managing the documentation of CHW and Health Navigator encounters within the data tracking system, upon obtaining the appropriate consent, to identify services, assist in navigating the healthcare system and support data quality in alignment with the state and federal funding guidelines.
  • The Coordinator/CHW will prepare and submit all required information and reporting to the funding agencies as required as well as any other information that may be required by funding agencies during the contract period.
  • Attend meetings and conferences as required by funding agencies.
  • Collect programmatic data and relaying information to stakeholders to inform programs and policies.
  • Building community capacity to address health issues.
  • Ensuring cultural mediation among vulnerable populations, communities, and health and social service systems serving vulnerable populations.
  • service systems serving vulnerable populations.

Qualifications:

  • Certified Application Counselor – Health Insurance Marketplace (must complete within 60COVID days of employment)
  • Certified Community Health Worker (must complete within 90 days of employment)
  • Certified/Registered Medical Assistant preferred
  • Valid Driver’s License and Insurance Coverage
