Preparing For Your First Visit

At Mid-State, we are committed to providing accessible, high-quality, whole-person care to support your health and wellness. If you are a new patient, we’re so glad you’ve chosen us as your healthcare home. Below is helpful information to get started with your care.

Appointments

To schedule an appointment, please call our office. Our patient services team will assist in assessing your needs and scheduling you with the appropriate provider.

We understand that unexpected health concerns can arise. Same-day care appointments are available at our 28 Main Street location—even if you are not a current Mid-State patient.

If you need to reschedule or cancel an appointment, please contact us as soon as possible so we can offer that time to another patient in need.

What to Bring and When to Arrive

To ensure your visit is as productive as possible, please bring the following:

  • A valid photo ID (e.g., driver’s license)

  • Your current insurance card

  • A complete list of your current medications and supplements

  • Any recent healthcare records or information relevant to your care

Please arrive at least 15 minutes prior to your scheduled appointment to allow time for check-in.

Medications at Your First Visit

At your first visit, your provider will review your medical history, current medications, and any previous treatments. This conversation helps us build a care plan tailored to your goals.

If you are running out of medication before your first appointment, please contact the provider who issued the original prescription. They may be able to provide a short-term refill while you transition your care.

If you are currently prescribed controlled substances, Mid-State providers are required to review your prescription history in the Prescription Drug Monitoring Program (PDMP). Depending on the findings, you may be asked to sign a new controlled substance agreement before continuing your medication.

Please note: Due to federal regulations, Mid-State providers are not permitted to prescribe medications containing tetrahydrocannabinol (THC), including medical marijuana.

Services to Support Your Visit

To make your experience as smooth and supportive as possible, Mid-State offers the following services:

  • 24-Hour Clinician Access: Mid-State patients have access to clinical advice 24/7. For urgent concerns outside business hours, call our office to be connected to the on-call provider. In some cases, you may be directed to seek care at your nearest emergency department.

  • Patient Portal: Securely message your care team, view visit summaries, lab results, and more. Ask any member of our team about how to enroll.

  • Language Services: Interpreter services are available at no cost. Please notify us in advance so we can make the necessary arrangements.

  • Prescription Coordination: Our Prescription Team can assist with medication questions, refills, cost-saving options, and transferring prescriptions.

  • Transportation Support: If you need help getting to your appointment, let us know. We may be able to assist with transportation through our van service or a community partner.

  • Form Completion Support: If you need help completing paperwork, call our office to schedule a time with a team member who can assist you.

  • Financial Assistance: Mid-State offers a Sliding Fee Discount Program, payment plans, and support with insurance enrollment. To begin the eligibility process, call our Patient Account Representatives at 603-536-4000, ext. 1404.

 

 

What to Expect at Your Visit

We aim to make your experience welcoming and comfortable:

  1. Check-In: Our Patient Services team will greet you, verify your information, and ensure all paperwork is complete.
  2. Clinical Intake: A medical assistant will bring you to the exam room, collect your health history and vital signs, and review your current medications.
  3. Provider Visit: Your provider will meet with you to discuss your concerns and develop a care plan based on your needs and goals.
  4. Check-Out: After your visit, our team will assist with scheduling follow-ups, lab work, referrals, and provide any necessary documentation.
