Patient Portal

What is a Patient Portal?

By signing up for our patient portal, you will have limited access to our Electronic Health Records system. We are allowing all patients who sign up to request and view an appointment(s), request a referral, request a medication refill, receive lab test results, view your health summary (includes medications, allergies, medical history and immunizations) and ability to email some of the staff with health questions or concerns.

Please print and sign the Patient Portal Consent Form and mail to our office or bring it in on your next visit.

Once your login has been activated by our office, you can click on "Patient Portal" at the top, right-hand side of each page. This will allow you to login and take advantage of all of these great features!
