What is Mid-State Health Online?
By signing up for Mid-State Health Online, you will have limited access to our Electronic Health Records system.
We are allowing all patients who sign up to request and view an appointment(s), request a referral, request a medication refill,
request lab test results, view your health summary (includes medications, allergies, medical history and immunizations) and ability
to email some of the staff with health questions or concerns.
Please print and sign the
Mid-State Health Online Consent Form and mail to our office or bring it in on your next visit.
Once your login has been activated by our office, you can click on "MSHC Online" at the top, right-hand side of each page.
This will allow you to login and take advantage of all of these great features!
