Mid-State Health Center strives to make accessing health care as smooth a process as possible.

INSURANCE COVERAGE

Mid-State accepts most major insurance companies, including Medicaid. For services such as dental, behavioral health, chiropractic, and other services not traditionally included in primary care, it’s always a good idea to call your insurance company ahead of time to find out if the service is covered. 

Below is a list of our most common insurance companies. If your insurance company is not listed, claims will be submitted. However, we cannot guarantee coverage or payment.  Patients are responsible for all outstanding balances not covered by insurance. Please contact your insurance company directly to confirm coverage.

SELF-PAY PATIENTS & UNINSURED

We are happy to work with self-pay patients! We offer assistance in accessing enrollment in the Health Insurance Marketplace. Self-pay patients who pay at the time of service will receive a 30% discount (discount is not available for dental services or sliding scale fees).

 

COST TRANSPARENCY

NH HealthCost was developed by the  New Hampshire Department of Insurance to improve the price transparency of health care services in New Hampshire.

Please visit their website to explore the expected costs for various services, depending on the service provider and type of service. This encompasses procedures such as MRIs, colonoscopies, mammograms, and various other health services, tests, and screenings.

Under the law, health care providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the bill for medical items and services. Learn more about your right to a Good Faith Estimate here.  

THE HEALTH INSURANCE MARKETPLACE

Outside of Open Enrollment, you may qualify for a Special Enrollment Period:

In the Marketplace, you qualify for a special enrollment period 60 days following certain life events that involve a change in family status (for example, marriage or birth of a child) or loss of other health coverage. Job-based plans must provide a special enrollment period of 30 days.     Mid-State is here to help! We have four specially trained Certified Application Counselors available to assist you.

    Certified Application Counselors are able to help you understand your options, help you apply on the Marketplace and can assist with questions you might have about your situation or any special enrollment issues.
